Liver Transplantation is not just an operation its a process and a beginning of a new relationship which is between the patient, the transplant team and the transplant institute. As we all know that, to maintain a healthy relationship we need purity, dedication and transparency from both sides – Health care workers as well as the patient and his family. And one should pledge to maintain this relationship. Shortage of organs, immediate finances, lack of pre and post transplant education, failure to understand pre transplant management in detail, pros and cons of liver transplant, life long medication, post transplant life, effect of transplant children and donor safety are the major patient issues. A new organ gives new life but only well-informed execution can optimize the chances of success to the satisfaction of patient and the family. It is important that the patient and care givers are fully guided and educated regarding the process to prepare themselves for this major undertaking. This precisely where the role of experienced coordinator matters most to facilitate solving minor and major problems that may creep up before, during and after the operation. This can be done through education and counselling sessions, group interactions and guidance. In my considerable experience, I have had occasions of dealing with the patients and families of the patients with liver failure who resign themselves to death even when some of them can be saved by timely emergency liver transplantation. The cost of liver transplant surgery in India varies between 15-25 lacs from institute to institute. Successrate of liver transplant is same in all the centres of India.

Liver transplantation is the only option for patients with decompensate end-stage cirrhosis and in many cases of Fulminant Hepatic Failure or Acute Liver Failure, also in patient with non resectable HCCs. Most patients return to a normal active life style within 3–6 months. After the first human orthotopic liver transplantation (OLT) in 1963, the procedure has become a standard therapeutic procedure world-wide. In India, liver transplant programs have been seriously obstructed by the lack of finances and cadaver donors. Seldom, by emotional decisions by the recipients by not accepting organ from their near-ones. Lack of cadaver donors in India limits the number of patients who can receive this life-saving procedure. Due to this problem Transplant surgeons in care of these patients have endeavoured to perform transplants with partial liver grafts from healthy, volunteer live do¬nors, particularly a family donor in India as per HOTA. Approximately 8500 living donor liver transplants have been performed worldwide. Scale of LDLT is much larger than Cadaver / LDLT surpasses cadaveric liver transplants by a remarkable margin. In last six years our centre has performed more than 500 LDLTs.
After working as a Senior Transplant Coordinator in renowned centres of Liver Transplant in India with reputed surgeons and with the experience of dealing with about 10000 liver disease patients and personally coordinating about 600 liver transplants in India, I started feeling that I should provide the benefit of my experience to all the patients in need for Liver Transplant and not restrict myself to a single Team, Centre or Area. In past Decade, India has become an attraction for Medical Tourism specially for Liver Transplant because of 'Low Cost and High Success Rate'.

Liver Transplant in India - A Liver Transplant Coordinator's Perspective.

Liver is the largest abdominal organ. Liver is situated on the right side of upper abdomen behind the ribs and is triangular in shape. Liver weighs about 1200 – 1500 grams in an average individual of 60-70 Kilograms, roughly 2% of body weight.

Liver though a single organ, broadly it can be divided into two parts – right and left hemi liver and 8 independent segments (each having its own blood supply and biliary drainage) functioning as a single organ. Unlike kidneys which are two different organs and situated separately on either side in the body; liver is a single organ. However it too can be divided anatomically in two or more parts.

Liver as a whole has one hepatic artery supplying oxygenated blood, a portal vein carrying blood from intestines to liver and a bile duct draining bile formed in the liver to intestines. All these three structures divide into two branches to supply and drain from two lobes of the liver. Blood from liver is delivered to heart via three hepatic veins for purification.

Liver is supported by ligaments; triangular and coronary ligament on either side and falciform ligament in the centre. Liver produces bile which is drained by biliary tree. Gall bladder is a reservoir for the bile lies on the liver bed, and is attached to bile duct. It regulates delivery of bile into intestines.

Liver is endowed with remarkable capacity to regenerate after division into parts. This is the basis of Live Related Liver Transplants, and the reason why live related liver transplant is possible.
FUNCTIONS OF LIVER

Liver is a powerhouse of body. It is the main organ of metabolism i.e. it involves, series of breaking down and making up of chemical reactive and generation of energy. If one were to compare our body to a train, then liver would be its engine.

Liver performs many important functions that are essential for a healthy body as follows –

Covert food into chemicals necessary for life and growth.
Process and remove drugs, alcohol and other substances generated in body that may be harmful.
Produce bile, a yellowish-greenish fluid that is stored in the gall bladder and secreted into the bowel. Bile is necessary for the absorption of fats and vitamins
Manufacture important proteins that are necessary for normal blood clotting and building muscle.
Maintain hormonal balances
Store important vitamins.
